Dear Lois,
If you are hoping to help moms to continue breastfeeding with your follow-up phone calls, then they need to be initiated much sooner than two weeks post discharge. By then most of the common problems have presented themselves and often lead to weaning if no help is available. We do F/U calls within a few days of discharge, sometimes the day after if significant problems have already presented during the hospital stay. After mom leaves the hospital, the assessment sheet we use on the floors is put into a folder (we have six - Mon thru Sat) and each day the calls are made. If more F/U is needed, the sheet goes back into a folder. Eventually, they are retired to a permanent file when mom feels she no longer needs our help. On the front of our sheets there is room for detailed assessment of mom and baby and on the back, there are abbreviated assessments for the F/U.
