As I recall from nursing classes a dozen years or more ago, they made this discovery when they started doing earlier ultrasounds, like the vaginal u/s. Those done in the early weeks showed a higher frequency of multiples than was reflected in newborn numbers. The way this was explained was that
the often unexplained first trimester bleeding experienced by many women was actually the loss of a twin. I know I experienced this first trimester bleeding with my oldest daughter and was told then, 25 years ago, that this was a fairly common occurrence. Perhaps others know of actual studies.
