Nancy,
Could this sensation be in the "Tail of Spence" (the portion of breast
tissue that extends into the axilla)?  Depending on the degree of pain,
maybe she just feels the let down in an area she doesn't expect to have
breast tissue?

With all three of my children this region became uncomfortably engorged when
my milk came in.  Additionally it didn't empty as easily as the rest of the
breast.  With baby #1 I thought I had breast cancer (a little overwrought as
a new mom), by baby #3 I'd figured out to do some gentle massage to "move"
the engorgement into the breast area.  I do not have a supernummary nipple
there, but almost wish I did as it would have been easier to remove the milk
in that area.
