I know I've posted on this in the past and don't have time to look it up
now (check archives in the last year or so under nipple confusion and my
name).  I feel the problem is that we are lumping several potential
difficulties under the heading "nipple confusion".  For every difference
between action/feel of breast vs bottle (or more accurately, human
nipple/breast vs artificial nipple), there is potentially a baby for
whom that difference can be a stumbling block.  For some it is "shape
confusion", for some "rate confusion", parhaps "texture confusion",
"flow confusion", "taste confusion", "temperature confusion", "where it
stimulates the mouth confusion",etc. etc.
