I think that like all good ideas and instinctive compensatory maneuvers, the
teacup hold has prob. been around forever.  I use it sometimes to help moms
with babies who have cleft lips.  The pinched up areola can be held in the
cleft and used to plug it up in order to improve seal and suction.
