Susan,
Let the parents know that Flagyl is prescribed to babies.  Next let them know
the issue around this medication has to do with the first trimester of
pregnancies and that there are no reports of any problems to babies who were
nursing and Moms on Flagyl.
Regarding the blood cultures.  Ask the doctor if he is concerned that the
infection will affect this Mom's brain.  What I am getting at is the
blood/brain barrier.  The breast act the same way to protect the baby as the
brain does to the rest of the body.  Because an infection is systemic does
not mean it goes into the breast milk unless the infection is in the breast
and even then it is OK to breastfeed.
Hope this helps and this poor Mom gets better soon and no one makes her wean.
