Good day, everyone.  I have already privately e:mailed some fellow
lactnetters, but given the recent topic of early termination of
breastfeeding and the mention of surveys that people have used or read
about, I thought I would post this to the whole list:

The question of duration of breastfeeding/exclusive breastfeeding is going
to be looked at by my hospital.  If anyone has a questionnaire/survey tool
they could share with us that addresses duration of breastfeeding/exclusive
breastfeeding and reasons for stopping breastfeeding or exclusive
breastfeeding, I'd really appreciate it.

Our breastfeeding committee will be meeting February 13th, so any
information prior to that date would be great!
