I am a hospital based RN and IBCLC. We are currently looking at updating
our charting and procedure for outpatient visits. I would appreciate input
from any of you. Who do you admit as the patient when you see a couplet?
The mom or the baby? Do you only send the charting to one chart in med
records, or do you send a copy to the mom's and baby's chart? When you bill
insurance for a consult, again, who is the considered the patient? We have
been seeing ouptatients for a number of years and currently have one
assessment form. We send the form to the baby's med records. We are
working to make sure our charting and procedure meet JCAHO standards. I
would appreciate any imput. Thanks.
