I've seen several women with layers of skin debris on their nipples.  Mostly
I've seen this postpartum when the skin flakes off, gets in baby's mouth and
spit up (which freaks everyone out).  One RN told a mom she was 'dirty'
because this happened, and the mother was in such terrible shame she could
barely function.  If the dead skin layers come off all at once, the
underlying skin is tender, like proud flesh.  I do recommend daily cleansing
of crusty debris from oozing fluids.  This can be gentle, in shower, with
soft toweling.  Present this as matter of fact information.  Not all women
have this build-up, but for the ones that do, it's just a hygiene issue.
