Cabbage leaves are not my first line of defense for engorgement. Rather, I
usually only suggest them when other measures have failed and/or the
situation is severe. When I do make the recommendation, it is for continuous
use *until the milk begins to flow*, and then discontinue use.
I picked up this protocol largely through lactnet discussions in past years.
Recently I noticed that some people are recommending only short periods at a
time due to the fear of milk supply disappearing. From what I can tell, such
limits have been based more in the fears that have been expressed about
losing milk than in actual fact or research. It seems to me that unrelieved
engorgement itself should be the major suspect in killing milk supply, and
that until we get the milk flowing again, the former concern is a moot
point.
