R/T your pt whose previous baby breastfed for only 5 days due to shooting pain in the top part of her breast and extreme afterpains: my question would be: What was the birth like? Was she induced? How much pitocin did she receive? Was she overhydrated so that the engorgement from the fluids (and the pitocin which is an antidiuretic) caused pain with MER? Also how long was she given Pitocin? This can add to pain from MER. Also, did she have uterine clots her body was trying to expel in those first few days? I've seen women cry with MER in the early days as their uterus became hypertonic with trying to expel clots or retained placenta. I believe you need much more information from this lady before you might make a guess as to the problem with the first baby. Also keep in mind, women who obtained an epidural early in labor are more pain sensitive after. Also smokers. Good Luck and I hope she is able to have a better time next time.
