In my work for WIC...I once asked a family what kind of formula their
(formula-fed) child was on...and they answered me.. shes on Pedialyte...
They had been feeding the baby grape flavored Pedialyte for DAYS...apparently
they weren't aware it wasn't formula and that they were supposed to return
back to formula after an interval.

This was many years ago..and I dont' recall the outcome of this, but we did
get that baby back on formula asap.
Some parents are scary.
I once had another mother feeding undiluted liquid concentrated
formula...because she noticed that her baby slept longer on it! *sigh*.  She
was angry at me for issuing her ready to feed formula.

And I had another well meaning mother feeding concentrated formula undiluted,
and the pediatician kept changing the formula from brand to brand because of
the vomiting and diarrehea and no one asked her about preparation.  She
thought the concentrate was ready to feed.  I had her bring the baby to the
emergency room immediately where it was treated for dehydration.

Its actually not uncommon for parents to purposely mix the formula
differently then the directions for different reasons.  I always ask parents
how they are mixing the formula.

...any wonder why I became a breastfeeding proponent??
