I've told part of this story already.  It's just a particularly neat one.

The mom was reassured by everyone that things were fine, and that she'd be
over the soreness soon.  Increasingly, she felt things were fine only
because she pumped for extra bottles to keep from going nuts.  Let-downs
were erratic, nipple was lipstick-shaped, weight gain was fine but mom was
an "easy pumper", readily collecting those extra bottles.  Gradually she
found a position, with baby on back, that improved let-down reliability and
nearly eliminated soreness, but feedings were still long and of
unpredictable satisfaction for the baby.

A marginal tongue-tie was clipped at 5 weeks, and baby had a day of
blissfully normal nursing - ten-minute feeds followed by satisfied sleep.
Mom was elated.  The next day it was worse than ever.  But as his tongue
healed, the baby began nursing effectively.  Then, within two weeks of
having his tongue clipped, he became extremely fussy again, throwing up,
screaming sometimes for a couple hours at a time, usually unwilling to nurse
unless he was just waking up.

Your guesses?  The key is in those words "easy pumper" in the first
paragraph.  Now that he could extract milk efficiently, mom's
eager-to-please supply apparently went over the top.  She didn't spray or
leak, and his diapers were normal, but his new behavior and the way it
coincided with his suddenly efficient tongue made oversupply seem like the
most logical cause.  She began spending 6 hours on a side... and finally had
an efficient baby with a happy tummy.  He's 4 months old now, and the only
time he spits up is sometimes after that first nursing on the full side...
