As the coordinator of a hospital based home visit program, I can tell you
that we frequently see babies on the 4th day of life with high bili's.  The
fact that we reduced the length of stay in hospitals without changing the
follow up is disturbing.  Babies are discharged in 48 hours, and aren't
schedule to see the pediatrician for 2 weeks.  A lot can happen in that
short two weeks!  All of our home visit nurses are either IBCLC's or CBE's,
and a large part of the teaching they do is breastfeeding related.  90-93%
of our patients accept a home visit from our program.  According to the
patient satisfaction evaluations we receive, our home visit program has
saved a number of breastfeeding relationships.  And that makes us feel good!
