A possible mechanism for this would be just as Debi says, that the babies
have not had effective technique and have done fine on the 'runoff' while
the breasts have never had the good, effective stimulus of a significant
drop in internal pressure from a thorough feed.  Perhaps it takes several
months for the milk glands to respond and react by reducing production
capacity, and due to the volume of the oversupply the problem has gone
unnoticed until supply drops below the critical point between baby satisfied
and baby NOT satisfied.  The question is whether the drop is irreversible,
but given that it has happened with each baby (starting out with lots before
tapering off) my hunch is that it could be remedied.  If mother can bear the
thought of an intensive week or two of all the old stand-bys for increasing
production, and can carry it off with 4 children in the house (maybe one of
them would volunteer to help :-)) we could see.
