I could use some help with breastfeeding documentation.
 About 2 years ago we adopted the BSC Breastfeeding documentation,  as away to start being consistent in the charting.
 That has worked well up until now, when we have old and new people slipping back into the habit of charting nursed 10 minutes on one side, etc.
 I know I have read someplace the pitfalls of charting this way, but for the life of me I can't remember where. Can anyone point me in the right direction of where or what I might have read???
 Greatly appreciate any comments or help.
