As regards the position, below, I know of another use for it.  If the mother
is too sick or otherwise unable to roll over unaided, she can feed the lower
breast (lying down) and then by leaning just a little further over she can
feed from the upper breast.  This was my only option when I had pleuresy,
following influenza, when my youngest was about 5 months old.  Moving was
agony and not to be contemplated, there was no adult there all day to help,
and the other children, the eldest 5 years, had to make themselves
sandwiches and get their own drinks of water.  I shudder to think what would
have happened to the baby had she been artificially fed!  Yet another
example in my own life of "breastfeeding to the rescue".
