I've used both personal letters and brief summary forms available from The
Lactation Institute, BSC and other sources, and have to say I prefer the
forms.  They force you to be concise and the docs like getting them in a
standardized format.

An additional bonus for me is that my forms come from America, making them
totally unique here.   Both doctors and their staff have told me the reports
get read because they are short, and they've never seen the form before!  So
you might want to design something unique of your own.
