I asked this same question of the group about 2 months ago and posted the
results last month. You could email me privately or check the archives. Since
that survey and a few group meetings we have decided against the nurse double
check system for now but are going to employ a few other safety measures. The
first is that moms will make 2 labels per bottle, one they adhere and the
other the nurse will place on a flow sheet and date, the subsequent labels
placed on the flow sheet will only be when a new bottle is opened, since
often you have several feeds from one bottle.  This will also allow us to
track meds better. The second part is that each bedside will have a sheet
that contains 80 small stickers with the name and chart number plus BM:
(The space beside the colon will be for fortifiers when used). A corner of
this sticker is put on the syringe/bottle while warming and being given, then
placed above the space where volume is written in the nursing flow sheet.
These extra precautions should give more focus on reading labels, which of
course is all one really has to do to avoid a mix up.
