I have researched that number to its origins.  It was based on a study done
in England which if I recall correctly was  estimated by taking the current
infant mortality rate looking at the causes of death that could be attributed
to formula feeding and then calculating this number.   This study was done in
the 70's.   Dr. Rogan  from the NIEHS used this number in the following
article.   Dr. Rogan informed that the abstract was ambigious and that he had
no intent of reconfiguring this rate.  In general England's mortality rate is
lower than in the US.  Given the way this rate was calculated, given that is
was done 25-30 years ago in England,  I am not so sure that this is a
relevant rate to this century in the US.  I have devoted a whole presentation
