I doubt anything is wrong.  This describes many babies of this age...I
recommend that mom learn to nurse while carrying baby *upright* in a sling or
baby bjorn - then I make a point of scheduling another time to see her so
that I can teach her how to do that.  Many babies at this point seem to be
'on the verge' of a nursing strike.....this 'on the verge' phase can last for
months.

Does mom frequently nurse baby while lying down when at home?  Perhaps baby
is used to that and doesn't care to be fed while sitting up being held in
arms.

I'd make the sling/carrier recommendation, then discuss sleeping with baby
for naps and nighttimes.  Baby can easily nurse enough during those times to
make sure that she gets more than adequate milk...and mom can get more than
adequate sleep. I also find that many times mom is resistant to sleeping with
baby because she wasn't adept at nursing lying down....so I show
her...regardless of where we are - we work out several good positionings so
that mom has options and ideas..
