Sorry this is such a late reply to this issue.
Marjorie Meyer Palmer (a speech and lang therapist who specializes in
feeding issues) states that at 3 mos of age, sucking moves from being
reflexive to volitional, and many babies who seemed to be feeding well
but were really hurting during feeding will suddenly refuse to eat at
this age. This jibes with our observations that infants dealing with
mom's hyperlactation or strong MER often start to refuse to bf at 3
mos.  In addition, I would see if baby had signs consistent with GE
reflux as well.  Babies are smart, and can associate the taste of milk
with their discomfort in feeding, and may refuse to take it in other
containers as well. I had a toddler on chemotherapy in my practice who
refused to nurse or take her mom's milk in any form (even frozen into a
popsickle) because she associated it with her nausea.  It took a week of
patient reintroduction to the breast after antiemetic therapy was
instituted for her to be willing to feed again.
