I have reviewed the article and have the following comments to make (please
remember that I have written review articles of a similar type and thus may
be biased in my comments):
1.  The article had an excellent, simple, easy to understand introduction
2.  The primary criteria for safely breastfeeding (generally) was that of
the infant being exposed to 10 percent of the therapeutic  dose is based on
information previously espoused by the author and is somewhat overdone, it
seems, to the exclusion of a lot of other available information
3.  Table 1 (drugs that require careful assessment) data was based on
information also previously published by the author and was fairly limited
in scope
4.  Drugs covered in each category in the article were quite limited
5.  Drug categories covered were very limited
6.  Nothing really new was added to previous reviews by other authors
7.  The statement in the conclusion that there is a dealt of epidemiologic
data on the probability of adverse effects in "breast-fed" infants leads one
to believe there is NO data available, which is not true
8.  Use of "breast-feeding" for "breastfeeding" really bothered me as it
seems to indicate an author who is not in that close of contact with the
real world of daily medications and breastfeeding consultation
Maybe I am just amazed that the NEJM actually published an article on this
topic.  As I have stated, I have seen much better review articles published
by other authors (including myself), but I guess we were intimidated that
the NEJM would not publish such an article (stupid!!!).
By the way, I have two new articles in press on over-the-counter medications
and breastfeeding and on herbals and breastfeeding in press.  I will let you
know when they are published.
