I got a call  from the mom of a 5 week old baby  who is just 2 ounces
over birth weight.  I have never seen a family work so hard, and
struggle so much with trying to breastfeed.  I believe they have seen at
least 3 different LC's in 2 different states, they are presently using
the SNS to give supplements, taking Reglan, and using a hospital grade
electric pump.  The mother reports that she has hypoplasia in one
breast, and feels that in spite of all her efforts, her supply is
actually getting less.  They visited another physician this week and had
HGC, Prolactin, Progesterone and TSH levels done.  The call back from
the physician was that the progesterone level is low, and she ordered
Crinone 8%  (90mg progesterone gel).  The question to me was how will
this affect the little milk supply she does have ???  I have checked
several resources and the archives - so far  - the mention of
progesterone other than in relation to birth control pills was to
indicate that the level drops after delivery , and to stimulate this
hormonal response may enhance proliferaton of the alveolar and ductal
systems but may inhibit lactation per se.  After this 5 week struggle,
the mother is willing to use the progesterone if it is in the best
interest of her health.  Anyone have any knowledge about this?

Another mother with severe PIH has been placed on "Aceon" - it is
supposed to be an ACE inhibitor.  After reviewing the information in Dr.
Hale's book, the Neonatologist is having the mother pump, but not give
the milk until the mom is off the ACE inhibitors.  This mom is also
simultaneously taking Aldomet, Calan, and Tenormin.  Anyone have
knowledge of Aceon?  Thanks for any help.  Evelyn Landry, RN, IBCLC
