I would suspect a tight labial frenulum (under upper lip).  These can be
clipped, but are more of a challenge than the lingual frenulum problems.

I had 2 mothers last year (within the same week) with this problem.  Both
mothers chose to feed in the upright, clutch hold and not have the frenulum
clipped.
