The most difficult and frustrating case I ever had happened last
September (with my own sister!)  Her baby was deep suctioned and handled
"rather roughly" at birth.
By the time I arrived in Washington D.C., 2 days after the birth, the
baby was either sleeping or screaming.  He wouldn't tolerate the breast
at all.  We fed him expressed milk by cup or with an eye dropper - small
amounts every hour or so.  It reminded me of feeding a wounded baby
bird.
Finally we tried "rebirthing" - putting the baby naked on the mother's
belly in a completely darkened room.  After about 1/2 hour the baby
crawled onto the breast and latched on and there was no turning back.
Since then I've tried it with other babies that have had difficult
births and have had good results.  Good luck!
