You could type up a professional report and send to the pediatrician. You
could add a cover letter stating that you are available for consults and ask
how best you can work with the pedi so that you can get to the moms sooner,
stating your concerns in a general way. Like by getting a consult very soon
you can protect a milk supply, prevent nipple trauma, wt loss, etc. Say it
in a generic way not just about this particular mom.
You could contact the maternal child director with the same type of letter
or phone call and ask to meet with him/her about community followup. You
could ask to meet with the LCs to coordinate community followup for at-risk
dyads. You just have to say that you are aware of the time constraints and
large pt load of hosp programs and that you would like to work together with
them to take up the slack of after discharge problems. Surely they don't
have a followup program if the mom never even got a phone call. Good luck.
