So it's 2 cases again!
The first one with hypothalmic radiation likely is primary lactation
failure, unfortunately. The pituitary gland or hypophysis is often called
the governing endocrine gland of the body and very important in human
reproduction. The mom is probably on DDAVP [Stimate, desmopressin acetate]
which is a synthetic antidiuretic hormone. See Hale's book - it should be ok
for bf. To help this mom, do all the usual milk supply measures, up to the
mom how far to go, and she can supplement with supply line like sns if she
likes.
The second case I don't know; I'm too tired from working outside in 98
degrees F and high humidity to think! Maybe the letdown is more forceful?
Does she usually nurse there first? Maybe she has a subclinical mastitis and
the milk is salty?
