You say the nipple has a crack, maybe there is much pain inhibiting the
letdown on that side. Maybe some of those remedies that are supposed to help
with letdown (rescue remedy - search archives) or a warm towel around her
neck and upper back or a massage. I agree I would advise continued
expression of that side, maybe hand expression or try a different pump or
flange. Since this is a one week old baby I would be hopeful. Address moms
anxieties, fears, maybe she needs peer support too.
As an aside, I wonder if the MER is inhibited and milk is not removed
wouldn't that side involute? I guess that involution can occur at different
rates in different women just as milk supply, onset of copious milk
secretion and all that is very variable as well.
