Thanks for the replies on my mom with long nipples.  This is third baby.
Others had weight gain issues and early weaning as well.  Normal pregnancies
and engorgement, leaking milk at the consult.  However mom states she can't
pump has tried everything from the lactina to evenflo.  I gave her a
PersonalEase flange and she did get a bit with it, so maybe it will work
better.  The latch looked ok and baby was not gagging but also wasn't
sucking right either.  The breast was so small at the areola very hard to
maintain a wide angle after latch.  Will follow up with mother tomorrow and
hope he is doing more nutritive sucking.  Mom's nipple and areola look a lot
like a haberman feeder if that helps.  I really think there will be milk
supply issues because storage capacity seems so small, of course I've had
small breasted women nurse with no problems at all but their breasts were
rounder.  Mom gets engorged on one side before each feeding.  Breast feels
hard to the touch.  Given that pumping is an issue for this mom,  how to
maintain the supply is an issue.  Will ask about hand expression tomorrow,
but with two toddlers, even the sns is seeming overwhelming to her.  And of
course I am leaving on a two week vacation on Friday.
