Hi everyone!!  Been a long time since I have posted anything but it is time!

I work in a hospital as a parttime  L&D nurse because my private practice
doesn't generate enough revenue for a single mom of 3 kids! The hospital
already has 1 LC that is doing a good job based on her constraints.  (250+
births/month, rounds and clinic visits).  I want to justify the need for
another LC.  They have asked me to create a customer satisfaction survey r/t
lactation services.

I am sure that there have been others already generated/created!  I thought
that you lactnetters that have already done this could email me with an
excellent form so I will not have to reinvent the wheel!  I don't have time
to "miss the point" on this because if I cant justify it then they wont
approve it!  Anything anyone can do is greatly appreciated and credit will be
given!
