The way a BF baby could be colonised and then infected is by putting
something in her/his mouth that had been contaminated with the bacterium.
For example, if a sibling were colonised and had traces of E.coli on hands
(from feces-- and soiling does not have to be visible to the naked eye to be
infectious) and touched the baby's face, or the breast, or if the mother
were contaminated when caring for a colonised sibling, it would be simple
for the baby to get the bug into its GI tract.
It is not possible for it to pass through breast milk unless the mother has
a breast abscess caused by that bacterium. Yuck!
