Dear Kay,
Do you know what organism is causing the infection? Has a culture and sensitivity ever been done? Baby may be harboring staph aureas or some other bug and reinfecting mom. May want to treat baby as well.
In these cases, it may be that mom should be on continuous low-dose antibiotics throughout lactation to stave off these infections. It may sound a bit overboard, but recurrent mastitis, including hospitalization, is worse. It is similar to what is done with children with recurrent ear infections - definitely way overused in the case of ears, but I think sometimes underutilized in cases of recurrent mastitis.
