In your post you reported that mom's nipple is white after feeding (besides
being painful).  It sounds like a baby with a strong clenching response.
Does the baby also exhibit hypertonia and arching away?  Is he difficult to
cuddle?

Page 70 in Breastfeeding Answer Book gives detailed description of this
problem (it used to be referred to as "tonic bite reflex") as well as
several suggestions for mom to help baby relax and not clench his jaws.
