Laurie, you don't mention whether anyone knows how much of the baby's
bilirubin is conjugated and how much is unconjugated. Granted this baby is
very jaundiced, but the weight loss is modest and the baby sounds more
lethargic and dehydrated than I would expect under the circumstances. Also
the stooling pattern sounds wrong somehow. Increasing amounts of DARK
stools to twenty a day before the baby is seeming vigorous? Not what I
typically see in physiologic jaundice.
I think your plan, to preserve the BF and feed the baby, is sound. I would
also want someone medical to really do a work-up and find out what is going
on besides poor feeding. GI reaction to the formula? Spherocytosis after
all (about which I know nothing)? Something else entirely?
