<<plastic surgeon that she "did not have enough breast tissue" to make milk.>>

we need to have this fortune teller at our next ILCA convention.
sorry, could not resist.

<<I'm hoping to give him some confidence in judging supply adequacy.>>

as you said, you will guide them in supply eval and frequent f/u will give
them confidence. you can keep in the back of your mind an sns prn.

<< Why is she leaking from the accessorys and not the actual nipple? >>

prenatally, many women do not leak from perfectly good nipples. maybe the
accessory sites are not the same tissue as regular nipple tissue, ie
erectile. more relaxed tissue would be more likely to leak - not as smart as
regular nipple tissue. the surgery may also be a factor.

the leaking could be very encouraging - she is making milk.

<<if baby has a wide latch that includes the accessory nipple sites, do you
think nursing on that side might be possible?>>

yes, and even if the latch does not include the accessory site. surely she
has pores in her regular nipples - for proof of this, after birth have
someone skillful massage and express some colostrum drops - mom may find this
reassuring also.

recently a multip mom told me that she had regular milk, exactly like the
milk from her nipple, squirt at least a foot outward, from a very small
areolar bump, with every let down. she had no other signs of accessory
nipples anywhere else. it was a laundry issue.
