It seems to me that the shorter, more frequent feedings make more sense for
this baby than the 2-hour feeding sessions  you described, in terms of
increasing the fat content, stimulating milk  production, etc.

Could the mother try a day or so of no supplement, and feeding the baby
every time he seems interested? Since his weight gain has been so good, he's
not going to get in trouble in just a day or two, and it may be enough to
get her milk supply up all the way. Of course, watch his diapers, etc.

Is it possible that he has a stooling problem unconnected with
breastfeeding? It does seem odd that his weight gain would be good and yet
he's not having bowel movements. I know that some breastfed babies can, by
four weeks, be going several days between bowel movements but I'm not sure
it applies if the baby is also getting formula - and you say this started
much earlier. I would be curious about other potential causes here.
