Janna,
Sometimes I see this in babies who are having trouble holding a bolus of
milk on their tongues for a controlled swallow.  If the baby has a mild
tongue-tie, it can prevent the tongue from grooving strongly enough to
contain the milk that just entered the mouth.  The baby is forced to
swallow whether he is ready or not.  Sometimes milk gets into the
airway, causing incredible pain.  Mom can help by leaning back when she
breastfeeds so baby is more prone, this moves the milk to the front of
the baby's mouth and allows the baby more time to swallow in a
controlled fashion.
