I forgot to mention that  the video is mentioned in the back of the booklet

I would also like to see the video from The Children's Mercy Hospital in
Kansas City.  Has anyone seen that one?

I also have a pamphlett from The children's Mercy hospital  titled
"Information of cleft palate feeding aids" and it is 4 brief pages describing
an obturator, what it is, the best time to make it (right after birth), how
it is made, who makes it, the cost, how it stays in the mouth, how long it is
left in, cleaning, how long it lasts before needing to be enlarged, when it
is not suggested (when there is no feeding problem) and when does the use of
it end? (when the palatal repair is done)

It does not give the hospital's address, but someone interested could contact
their cleft palate team  in Kansas City, Mo.
