The Flexishield is an Ameda product.  It reduces the size of the breastshield to 21mm, I believe.  It would not fit in her 21mm Medela shields, but would probably fit in the 24mm.

In my experience, some women can't go by the descriptions of how pump flanges are supposed to fit.  Their nipples may be filling the tunnel of the shield but still pulling in more breast tissue than recommended.  The drawings on the M****a site are useless.

The Pumpin' Pals may be a better fit, but only because they don't seem to strangle the areola like traditionally shaped flanges do on some women.  They will, however, pull in more breast tissue.  The medium (smallest size) Pumpin Pals is said to be 23-29mm.  It will fit her Swing pump.

If this mother is having pain when pumping, she might start by simply turning down the suction.  Maybe that's why it is pulling in so much breast.  She could also try a little olive oil inside the flanges.  Have you watched her pump?  Did you think the 21mm was too big?
