Leslie, I would strongly recommend supple cups-- right away. This does not touch the low supply issue, but has a good chance of at least getting those nipples out further. The stretch marks may be a red flag if accompanied by poor breast changes during pregnancy. If mom really wants to be proactive, she could ask for a prolactin check to verify that she has normal pregnancy levels heading into delivery. She will need lots of TLC as she walks through this, and perhaps preparation to the effect that breastfeeding is not an all-or-nothing venture. I am working with two mothers who have great looking breasts, great veining, great changes.... one 80% supply, the other 20% supply. First mom's prolactin totally WNL, second mom was low (but not horribly so) and makes her 20% taking domperidone.
