Hello Betty, Welcome aboard. At our hospital we wear scrubs. I work in the NICU and on the post-partum floor. Sometimes we are called to other areas, ER, surgical floor, mom pumping, etc. We find that it is the most reasonable uniform for us. I am sometimes kneeling on the floor at a chair in the NICU. I carry a garden pad for my knees, and find this position saves my back. It also gives me a good view of the baby at the breast. We do not use a separate release form. The patient signs a release form when admitted to accept care from hospital personnel. For charting we actually use our hospitals computer set up. We have a form for "Breastfeeding Consult" which has a check list for teaching and education. There is also a space for a narrative note. This was set up when the charting system was set up.
