Helpppppp, I had a patient come in today with severe clicking when nursing at the breast. Assessment showed good latch. good nipples except left red and tender after baby finished but also left breast sl. engorged and that caused the nipple to be flatened, however when baby on right breast which was soft she made the same noise. Milk was very abundant. Baby weighed 5#15 at birth 3 weeks ago and now weighs 7#2. Had another IBCLC here at hospital come and take a look and we are both puzzled. What have I missed?Mother is going to pump before nursing with a Lactina and relieve the aerolar edema on the left but that won't make a difference on right since is not engorged. When baby finished on right side nipple looks normal shape but clicking is very loud during the whole feeding. I have seen clicking when swallowing but not when actively nursing. You can hear good milk transfer. Any suggestions?
