It sounds like she has "normal" breast involution. Some women find their
nipples or areola "sink" in  just a little bit after weaning. The skin may
be saggy, I  think because the fat in their tissue was replaced by working
ducts and now the ducts have involuted. This is what happened to me.
Although by the time I hit 40 years old, the fat was back due to normal
female changes called perimenopause  (due to slight weight gain)!

I can see how this would worry her.

I don't know anything about the creams but I have looked at some of them.
Going on the birth control pill, which mimics pregnancy can also make a
woman's breasts swell.
