I have permission to post the following very puzzling case. A mother has been BF a 4 week old without difficulty. The other day mother notices infant becoming fussy at the breast coming on and off. Suspicion of thrush was dispelled by two pediatricians who refused to treat. Infant was beginning to scream now after BF which stressed out the parents who decided to try formula to see what would happen. Infant did fine with the formula. Next feeding mother puts infant back on the breast and again the screaming but now infant has a rash around his mouth and has developed a diaper rash. Diet was discussed at length. She had already cut out dairy, peanut products etc. Takes no medications. Has not changed soaps, detergents, deodorant or anything along those lines. Finally got down to looking at possible environmental causes: pet sprays, lawn sprays, carpet cleaners. Nothing there either. LC then looked at where the father works for some possible contamination. None found. We suggested Mom try giving the breastmilk in the bottle which she did and the infant was screaming after that too. Infant was also checked for an ear infection and they were clear. The parents are totally baffled as am I and my fellow IBCLC. Mother and Father are committed to BF and very much want to continue. I am hoping that the collective knowledge on Lactnet may provide an answer.
