I think that the default assumption that population control always means "limiting third world population" is one of our (US and in this case a bunch of us Lactnet readers) deeply ingrained bad habits -- among other things its the motive for so many US physicians to not care about the WHO Code, because they just can't see it as applying to them and their patients -- "oh, that stuff's about the third world, right?"
