 Miriam:  I can think of two more possibilities for chronic plugged ducts:
ductal yeast (the filaments literally clog up the ducts) and sleeping on
her stomach (sort of like a tight bra would).  I'd go back over her history
and see if she's been given antibiotics and/or steriods.  Also ask about
sleeping position.   Besides lecithin, niacin has been successful for some
women.  'Just some thoughts...
