Re baby's teeth causing painful feeding for Mum:
Don't forget that when babies cut teeth, many have a change in pH of their
saliva - more acid - so that it irritates nipples. This in conjunction with
physical abrasion of the teeth can cause significant pain. To help this,
Mum can rinse nipples after feeds with a weak solution of baking soda
(about 1 teaspoon to cup of water), or salty water, to rinse off saliva.

This, together with scrupulous attention to positioning (which tends to get
ignored after initial weeks for a lot of nursing couples), is often
sufficient to solve the problem.
