I developed a breastfeeding survey that I mail to patients after I make
the follow up phone call. It has approx 8 questions on it . When a pt is
admitted , she fills out a teaching needs sheet, after discharge this
sheet is saved and the reverse side of it is the call back sheet. This
isde has moms name and address, phone number, her doc and babies docs
names, birth weight and d/c date. ALL of out moms get a follow up call
after d/c reguardless of how they are feeding baby. I make all of the
calls to the bf moms.  Ill bring these 2 forms home and try and scan
them . I dont have time to mail them to dozens of ppl at this time......
