I ordered some in the past, and I had them shipped by regular mail, very
reasonably, and within a very few days. It all depends on which postal
zone you live in, as it does when we mail packages within the US.

The disclaimer quotes the nearly $20 shipping fee [when their preferred
shipper is used]. I think they are referring to fast shipping  by someone
like UPS or Fedex, etc.

One line states very clearly: "Shipping by regular mail is most
economical. The cost will be confirmed by e-mail before shipping." This
allowed me to know just exactly how much would be charged to my credit
card, and to change my mind if desired, etc.

I can tell from the literature (and the website) that part of the problem
may be the difference in idiom  of someone for whom English is a second
language. I think the wording may be what is confusing.

There is also a toll-free # for ordering, and questions could probably be
cleared up easily as to shipping costs. Remember, the company sells many
products, like kitchen appliances and the same order form is used for all
items.

I intend to buy 6 more or so, and have them on hand for when I find a
mother for whom they will be helpful.

(However, I will not provide them when I think they are unneccessary. I
had someone call me today for one, simply because the baby is used to the
shield, which was essential during extreme engorgement. Now, the nipple,
while technically everted, does not stand out as far as the projection on
the shield, and so grandma was hoping the nipple puller would make them
stand up "tall enough" to stimulate rooting".

I just recommended a little time, patience and more skin to skin contact
after the shield was used to start the feeding.

I am really impressed with the tiny gadget, and have just made sure to
give good cleaning directions to the mothers to whom I have supplied
them.
